WAIVER & RELEASE OF LIABILITY

You wish your child, or minor for whom you are legal guardian, to participate in classes, performances, and other activities with SSD&G. In doing so, you acknowledge and attest to the following:

We love dance and gymnastics, and we’re here to provide an enjoyable environment for children to learn. However, as with any physical activity, there is always a possibility of injury. You understand dance/gymnastics poses risks minor and serious, and however unlikely, the possibility of disabling or fatal injury. You accept those risks on behalf of yourself and your child. You waive and release SSD&G from any liability from injury or harm occurring during or stemming from activities associated with SSD&G.

You attest that your child is in good health and does not suffer from any physical, medical, or other conditions that limit her or his ability to participate in any activities with SSD&G.

You understand that SSD&G’s instructors and personnel are not doctors or medical professionals. Nothing an instructor or other personnel says is or is intended to be medical or health advice.

Many activities at SSD&G will involve music. We select music that is fun and child appropriate. We use music from a variety of sources, including holiday music – some of which may include references or origins that relate to specific religions. It is not our intent to endorse any specific religion by doing so. By signing this, you acknowledge you do not object to this practice.

SSD&G is dedicated to make you and your child’s experience a safe and enjoyable one. However, as previously stated, any physical activity carries inherent risks. You waive and release SSD&G from any liability caused by the action, inaction, or negligence of its instructors and other personnel, on behalf of yourself and your child. Should your child become injured, SSD&G personnel will make efforts to assist. However, you understand we are not trained medical professionals and you waive and release SSD&G from any liability for harm your child sustains through our attempts to assist your child, or for any consequences from decisions we make on you and your child’s behalf should you be unable or unavailable to immediately do so. You understand you may be legally responsible for any harm or loss your child causes to another during the activities. You waive and release SSD&G from all liability related to you or your child’s actions or inaction, and the actions or inactions of others. You agree to indemnify SSD&G for any losses, damages, and attorney’s fees it accrues as a result.

SSD&G provides areas where participants can place personal belongings during classes and other activities. You understand that SSD&G is not liable for any loss or damage to any personal property.

During activities, you understand that SSD&G may take photographs, take videos, or capture other likenesses of you or your child. You grant SSD&G an unrestricted license to use such images to chronical, promote, or advertise its events and services.

All waivers and releases in this agreement are made on behalf of yourself, your child, and any other individual or entity entitled to make claim on behalf of that individual or entity, or on behalf of your child.

You certify that you are over eighteen (18) years of age, legally responsible for the child listed below, and possess the legal capacity to enter into this agreement.
